Please ensure that you have a copy of your ARCP Outcome 6;  you will need to upload this when indicated before your M4 form can be submitted. As well as this M4 Form, please ensure you have submitted to FOM your bound dissertation.  We will then forward a recommendation to the General Medical Council (GMC) for you to be awarded a Certificate of Completion of Training (CCT).  You can then apply for entry on to the Specialist Register.  Please note: it can take up to six weeks to make the recommendation once we have received all your necessary documentation.
