Please complete this application form in full and submit it to the Faculty of Occupational Medicine along with the relevant payments within one month of notification of your national training number (NTN) from your Postgraduate Deanery. Please provide as much information as you are able to; any missing information may delay completion of your registration. You do not need to complete the form in one sitting; you can close your browser and return to the form at any point. Once you’ve logged in, the form will be pre-populated with any information you have already provided. Once you have submitted the form, you will not be able to edit it and resubmit it. BEFORE YOU CAN REGISTER You will be asked to upload a signed declaration from your supervisor; you cannot complete registration without providing this. Click here to obtain a copy of the supervisor declaration; we would advise you to ask your supervisor to sign it before you begin completing the form. Registration in specialty training will require payment of a one-off registration fee when you have submitted the registration form, and an annual membership fee, which will be calculated on a pro rata basis depending on when you register with the Faculty. You will receive an email after you have paid your registration fee with instructions on how to pay your membership fee. You will receive a welcome pack with further information about specialty training once you have paid both your admission fee and your membership fee.
